Follow the link below to retrieve the mandatory player registration paperwork. There are two documents: one for Wisconsin Triple Threat registration, and one for Boys and Girls Club registration. These forms need to be filled out in full and returned in person oremailed to wisconsintriplethreat@gmail.com.
